Each General Information Sheet corresponds to the procedure you are interested in. Please print and fill out one of the following: Head and Neck InformationSheet, Hand Information Sheet, Breast Information Sheet, or Leg and Lower Extremities Information Sheet. Please print BOTH pages of the General Information Sheet, the second page is intended for Dr.High's use during
your consultation.

Please fill out each sheet completely; this will allow the doctor and our staff to have all the information to make your appointment as smooth as possible.
